Pop-Up COVID-19 Vaccine Clinic

What: Pop-Up COVID-19 Vaccine Clinic at the Aga Khan Park and Aga Khan Museum.

 

When: Saturday, May 15, 2021

8:00 am until vaccine supply runs out (2,500 doses)

 

Where:

The Aga Khan Park and Museum

77 Wynford Drive

M3C 1K1, Toronto, ON

 

The clinic will be located at the Aga Khan Park, just outside of the Aga Khan Museum. Entry is via the main Park and Museum entrance (by car or foot). Surface and underground parking are available free of charge. 

 

Who:

Adults aged 18 years and over who live or work in ANY hot spot postal code in Toronto are eligible for vaccination.  

Priority access will be given to those who live or work in the M3C and M4H postal codes, as well as seniors (age 65 and up) who live in ANY hot spot postal code in Toronto. 

This pop-up clinic will accept walk-ins and no prior appointments are required.  

Please bring the following with you when you arrive: 

• If you live in a hot spot postal code in Toronto: proof of address, such as driver’s license, bank statement or utility bill

• If you work in a hot spot postal code in Toronto: proof of employment, such as work identification or pay stub

 

Please also bring your health card. However, health cards are not required if you do not have one.

- Face masks are compulsory and all health and safety guidelines must be adhered to while on the property.

What if I'm not eligible for a vaccine, where should I go?

What if I have an expired health card, what should I do?

You do not need a current health card to receive the vaccine.

 

What if I have no health card?

You do not need a health card or OHIP to receive the vaccine.

 

How do I know what vaccine I will receive? Do I get a choice?

The pop-up clinic will offer Pfizer or Moderna, this is dependent on supply. 

 

What if I’ve had my first vaccine somewhere else, can I receive a second vaccine at your clinic?

No. Please wait for guidance from your local health unit or the location of your first vaccine. 

 

What if I have side effects as a result of the vaccine, what should I do/who should I contact?

Please monitor closely. If symptoms persist or worsen, immediately go to your nearest emergency room. 

 

How much does the vaccine cost?

The COVID-19 vaccine is free to everyone who is eligible

 

How do I get to the Aga Khan Park and Museum?

The Aga Khan Park and Museum are located at 77 Wynford Drive, M3C 1K1, Toronto, ON.

 

BY CAR

The Park and Museum are easily accessible from the Don Valley Parkway (DVP). Please take the Wynford Drive exit.

 

PUBLIC TRANSIT

The Park and Museum are easily accessible from several TTC stations including Pape, Eglinton, and Broadview. Please visit TTC.ca for the latest schedules, routes, and maps.
